    1. If a center based family does not have coverage, the questionnaire will be forwarded by the teacher to the Family Advocate (FA) who will provide followup with the family and set goals when needed to assist the family on the Medicaid application process to be documented in the “Health Information” tab, under medical/dental coverage notes.
    2. For home based families without ongoing source of coverage the HV will set goals when needed to assist the family on the Medicaid application process to be documented in the “Health Information” tab, under medical/dental coverage notes.
    3. The family will be given information on Medicaid (https://medicaid.ohio.gov) including managed care options provided by the HSM.

  2. During the second parent teacher conference, the teacher will review the “Source of Health Care Update Questionnaire” to ensure the family has not lost or changed coverage.  Home Visitors will review during the month of May.

    2. Home based staff will forward all changes or concerns to the FA for followup.
